Student Engagement Form - ACES Faculty

This form is for ACES Faculty, Instructors, and Staff that have concerns about the engagement of a student in their cours(es).  For example, if a student in a class was attending and participating in the class, but has now been absent or stopped submitting coursework, this form can be completed to report this concern to the ACES Office of Academic Programs.  ACES OAP will then reach out to the student and assist in the situation.

This form does not take the place of the "Refer a Student" form and process through the Dean of Students.  If you have specific behavior concerns that a student is exhibiting, or a student has disclosed personal information (suicidal ideation, substance abuse, depression, etc.) you can refer the student to the Student Assistance Center using the "Refer a Student" form here:
